Position Summary: 
Acts as a unit manager, completes MDS on unit, coordinates resident care, communicates between departments, families and outside agencies. Supervise and direct Nursing staff to carry out quality care. Supports admissions for skilled care and back up to Assistant Director of Nursing. Leads clinical care programs and shares on-call responsibilities.

Fundamental Job Duties and Responsibilities : 
1. Plans, organizes, directs, and evaluates nursing care of all shifts.
2. Defines, interprets, and evaluates nursing practices.
3. Assists with developing policies and procedures, assist with distributing information to staff members of changes or additions in policies or procedure.
4. Evaluates routines, activities and personnel on all shifts.
5. Coordinates comprehensive assessments, coordinate and helps with MDS completion, residents staffing, annual and quarterly care plans.
6. Works with Social Services for follow up on families, resident concerns, etc.
7. Establishes and attends unit meetings on a regular basis.
8. Recommends staffing patterns to DON.
9. Oversees unit orientation of new employees. Provides retraining of employee if necessary.
10. Works with Charge Nurse to monitor staffing patterns to ensure that code and resident needs are being met.
11. Supervises and directs activities of staff, reinforces in-service training and ensure staff are aware of any new procedures.
12. Completes performance reviews/evaluations of unit staff.
13. Ensures continuity of care of residents.
14. Review orders and ensures care plans have been updated.
15. Coordinates and ensures care cards follow care plans of residents.
16. Meets with other managers and maintain consistency of care for all units with approval of DON.
17. Maintains programs and systems of care in conformance with State and Federal regulations and guidelines.
18. Ability to operate medical equipment as need arises and in-service staff on use.
19. Oversees collection of data for Quality Assurance Program and ensure follow up with corrections as needed.
20. Monitoring staff and procedures for proper Infection control, Standard (Universal) Precautions being followed.
21. Responds to emergencies.
22. Assure residents assessments are being done with appropriate follow up.
23. Monitor resident diagnostic information.
24. Coordinates TD screening and monitoring.
25. Attends AM report, Supervisory meetings, and general staff meetings as scheduled.
26. Continues to keep current with standard accepted Nursing practices, Long Term and Rehab Care trends, clinical practices and supervisory skill.
27. Positive and cohesive working relationship with all departments.
28. Supportive of Nursing Home Administration.

Qualifications:
1. Current RN license with State of Wisconsin.
2. Two to four years nursing home experience.
3. Two years supervisory experience.
4. Ability to add, subtract, multiply and divide, calculate decimals, and percents and make use of the principles of descriptive statistics.
5. Ability to comprehend and interpret a variety of documents including: MDS, RAPS, behavior programs, rehabilitation assessments, using care plans, State and Federal guidelines, etc.
6. Demonstrates excellent interpersonal and communication skills.
7. Demonstrated organization skills and flexibility.
8. Demonstrates ability to work independently and adapt to change.
9. Strong PC Skills, data entry, database system and Microsoft Office.
10..Excellent communication skills and maintain professional behavior at all times to deal with a wide variety of people and ability to multi task.
11. Wound Care Certified preferred.
