We are currently looking to fill the following positions: (2) Part Time (7.5 hours / week) - AM Shift - Every other weekend Part Time (7.5 hours / week) - PM Shift - Every other weekend Part Time (11.25 hours / week) - 3rd Shift - Every other weekend Part Time (18.75 hours / week) - AM Shift (2) Occasional Status (1 weekend / month) Pay is commensurate with experience. Applications can be obtained online at www.fdlco.wi.gov Examples of Duties: · Provides quality, nursing care to residents under supervision of the charge nurse in a teamwork style. · Follows directions of nurse. · Assists with or provides personal care to residents. · Assists residents at mealtime and with nourishments and records intake. · Encourages, stimulates, and assists residents toward improved functioning, self-care and independence. · Assists residents with mobility, ROM, transfers, change of positions, encouraging residents to meet their individual rehabilitation potential. · Assists residents with labeling and storing of personal belongings and recording changes of personal belongings. · Responsible for housekeeping tasks affecting resident care such as change of bed linens, collecting and bagging soiled linen, making and cleaning beds and beside stands and other furniture, cleaning up spills, preventing infections, wiping off counters, keeping drawers and cupboards straightened. · Answers resident call lights. · Assists resident with elimination needs and assists nurse in collection of specimens. · Intervenes to decrease residents' agitation. · With nurses' direction, assists in applying protective devices and checks residents as indicated. · Does one-to-ones or group activities with residents. · Provides a safe, therapeutic environment that protects residents' rights. · Documents appropriately on records and continually updates nursing assistants care cards. · Escorts and supervises residents to appointments, activities or outings. · Acts as team member with other disciplines in meeting residents' needs. · Communicates with nursing staff, related disciplines, families and visitors as needed. · Takes T-P-R and B.P., if trained, and patient's weights. · Reports to charge nurse significant resident findings promptly. · Observes, reports, and records resident's physical and emotional changes. · Utilizes nursing assistant care cards when reporting off duty to nurse. · Maintains good work habits. · Assists in meeting resident needs to ensure compliance with HSS-132 regulations. · Maintains neat appearance in accordance with dress code. · Responsible to be at work as scheduled, on time and leave only when shift has ended. · Maintains a positive mental attitude toward self, job, and team members. · Maintains sensitivity to residents and in meeting their goals as indicated on care plan. · Follows facility policies, rules, and regulations. · Follows the State HSS-132 regulations and all the facility's policies, rules, regulations, and in-service training requirements. · Provides a safe environment and insure safety on assigned unit for all residents and staff by reporting any hazardous or potential danger to supervisor. · Is free of communicable disease and TB. · Pursues educational growth and development. · Attends all mandatory in-services and nursing assistant meetings. · Updates knowledge by reading memos, policies, procedures, and other educational offerings of facility. · Performs all related duties as directed. Typical Qualifications: Must be at least 18 years of age. Registered as a state and federally certified nursing assistant. Graduation from high school or a GED. Willingness to actively participate in in-service education classes. State of Wisconsin valid driver's license. Required Experience
