ST. AGNES HOSPITAL The RN Care Navigator – Behavioral Health is a specialized health care professional who follows patients across the care continuum and who serves as a single point of contact for referring physicians, patients, and caregivers. The individual acts as an educator for the patient and family, a guide in ensuring patients receive appropriate follow up and follow through for planned and urgent needs, and an advocate for the patient and family addressing their concerns and questions. The RN Care Navigator – Behavioral Health works closely with inpatient and outpatient multidisciplinary teams as well as community partners to ensure seamless access and care benefitting patients, participating physicians, and family members. BSN preferred. Licensed or eligible for licensing in the State of WI as an RN. Previous Behavioral Health knowledge, skill, and training is preferred (i.e., knowledge or skill in management of psychotropic medications, behavioral health modalities and interventions, and/or suicide assessment and prevention techniques). Two years clinical experience in area of assigned specialty is required. Prior care management experience preferred.
